To receive $50 off any services performed:

1.  Call us at (417)859-0711 for your first appointment.

2.  Print this screen.

3.  Fill out and print the "New Patient Form" (the link is to the right).

4.  Bring both to your first appointment.

You may choose to apply the $50 for services at a later date, but the printout of this screen and the New Patient Form must be presented the day of your first visit. 

This is not redeemable for cash, may not be combined with any other offer, and is not transferable.
